payment terms

Payment is expected at the time of service in the form of personal check, White Dove Massage Therapy, LLC gift certificate, credit card (VISA, Mastercard, AMEX, and Discover, HSA cards, FSA cards) and cash. If you can pay with check or cash, it will help me save on the credit card processing fees. If you pay by cash you will receive a $3.00 discount as a courtesy.,

Late policy

All appointments will begin and end on time. If you are late for your appointment, you will forfeit the missed time. This is so that the appointment following you is not impacted. If there is room in the schedule and it works for the client and Massage Therapist, the time can be extended, in some instances.

Client Health History form

All clients are required to fill out a Client Health History form. This information is kept confidential between you and your Massge Therapist. You need to keep your Massage Therapist updated on any changes in your health. This helps the Massge Therapist be aware of any medical conditions or requirements you have. It helps ensure that you receive the best massage customized to your needs. This information also alerts the Massage Therapist to any conditions for which massage therapy may be not suited, that is contraindicated

Referrals

If you refer a friend or family member to me and they book and complete a 50 minute massage, you will receive 15 minutes of massage time. The 15 minutes can be added to your session or you can save up to a maximum of 4 sets of 15 minutes to get an hour free session. The thank you minutes are subject to scheduling requirements. You need to request at the time of scheduling to use your thank you minutes. This is to ensure that the appointment following yours in not impacted.

24 hour notice required to change appointments

If you can't keep your scheduled appointment, you must give 24 hours notice or you will be charged for the full charge for the massage service. Its important to remember that you may be impacting another client's ability to get an appointment time. Your appointment reserves the specific date and time for you and it is difficult to fill the slot on short notice.

Any illicit or sexually suggestive remarks or advances made by the client toward the Massage Therapist

will result in the immediate termination of the session and the client will be liable for full payment of the scheduled appointment.
